GO-VAXX Clinic at FCT

In collaboration with the Ontario Provincial Mobile Community Clinic Program, the Filipino Centre Toronto is hosting a GOVAXX Mobile Clinic to provide people a safe, reliable and convenient option to receive their COVID-19 vaccination. 

The GOVAXX team will be providing mRNA COVID19 vaccine for 1st, 2nd, 3rd (booster doses) for adults and youth ages 12 and up, as well as the pediatric Pfizer COVID19 vaccine for children aged 5 five to 11, 4th dose boosters to eligible persons are also offered.

REGISTRATION 

We request all patients to book an appointment 2-4 days in advance of the clinic date at 8:00 am. To ensure timeslot, book an appointment either via:

1. Complete this short Google Form & bring your health card on site: FCT vaccine clinic - registration form  
2. Provincial booking website https://covid-19.ontario.ca/book-vaccine OR by calling the Provincial Vaccine Contact center at 1-833-943-3900.  Please be sure to mention the GO-VAXX when scheduling an appointment.
